Nurse Practitioner/ Physician Assistant – In Home Assessments – San Jose, CA
Base pay range: $/yr – $/yr
Conduct in‑home assessments 3 days a week (with some outpatient clinical work) and work from home 2 days a week. Explore and manage complex and chronic care needs of senior patients through interdisciplinary teamwork.
Responsibilities- Conduct in‑home assessments, including comprehensive history, physical exam, medication review, and appropriate cognitive/depression/safety screenings.
- Perform laboratory specimen collection in the home setting when appropriate.
- Identify diagnoses for active medical and care management of patients, focusing on chronic disease management.
- Communicate assessment findings to primary care providers to address care gaps.
- Provide patient, family, and caretaker education, emphasizing close monitoring and follow‑up of patient needs.
- Maintain knowledge of community resources for referral.
- Comply with HIPAA regulations and protect PHI.
- Engage in end‑of‑life care discussions as needed.
- Experience: One (1) year of prior clinical or home care experience preferred; prior EMR and geriatric care experience also preferred.
- Education: Master’s degree from an accredited NP or PA program.
- Licensure: Active California state Nurse Practitioner or Physician Assistant license; board certification. NPI Number, DEA, Valid BLS, valid California driver license, and current automobile insurance.

- Physical Requirements: Regular standing, walking, reaching, lifting and moving up to 20 pounds; clear vision is required.
- Yearly bonus up to 7.5% of base salary.
- Restricted Stock Unit grant up to 15% of annual salary.
- Continuing education reimbursement up to $1,500 per year + 3 CME days.
