Position: Nurse Practitioner/ Physician Assistant - In Home Assessments - San Jose, CA

Nurse Practitioner/ Physician Assistant – In Home Assessments – San Jose, CA

Base pay range: $/yr – $/yr

Conduct in‑home assessments 3 days a week (with some outpatient clinical work) and work from home 2 days a week. Explore and manage complex and chronic care needs of senior patients through interdisciplinary teamwork.

Responsibilities
  • Conduct in‑home assessments, including comprehensive history, physical exam, medication review, and appropriate cognitive/depression/safety screenings.
  • Perform laboratory specimen collection in the home setting when appropriate.
  • Identify diagnoses for active medical and care management of patients, focusing on chronic disease management.
  • Communicate assessment findings to primary care providers to address care gaps.
  • Provide patient, family, and caretaker education, emphasizing close monitoring and follow‑up of patient needs.
  • Maintain knowledge of community resources for referral.
  • Comply with HIPAA regulations and protect PHI.
  • Engage in end‑of‑life care discussions as needed.
Job Requirements
  • Experience: One (1) year of prior clinical or home care experience preferred; prior EMR and geriatric care experience also preferred.
  • Education: Master’s degree from an accredited NP or PA program.
  • Licensure: Active California state Nurse Practitioner or Physician Assistant license; board certification. NPI Number, DEA, Valid BLS, valid California driver license, and current automobile insurance.

    • Effective communication with patients and team members.
    • Problem‑solving, organizational, and time‑management skills in a fast‑paced environment.
  • Physical Requirements: Regular standing, walking, reaching, lifting and moving up to 20 pounds; clear vision is required.
