Full-Time Licensed Practical Nurse (LPN) – Home Health – $35-40/hr
We are a small business with a big personality, seeking a dedicated LPN who is flexible, hard‑working, creative problem solver, and willing to roll up their sleeves to care for our clients.
Job Responsibilities Rapid Response & Readiness- Flexible 40+ work hours during Monday‑Friday 8:00‑20:00, some weekend shifts; occasional overnight shifts if required.
- Be ready to leave for a shift within 30 minutes during on‑call hours.
- Maintain readiness: keep uniform & , childcare arranged, cell phone charged, transportation available, equipment & supplies accessible.
- Support office operations when not on field shifts.
- Conduct initial assessments, re‑assessments, and safety evaluations.
- Deliver skilled nursing per Ohio Nurse Practice Act (infusions, wound care, injections, etc.).
- Observe and report changes in client condition, documenting thoroughly.
- Follow infection control protocols and universal precautions.
- Respond in emergencies, using equipment safely.
- Develop & maintain person‑centered Plans of Care; obtain physician signatures.
- Coordinate care with families, providers, internal staff.
- Assist with referral intake & follow‑up communications.
- Assist Clinical Director with weekly chart reviews for compliance, incident reporting, timely discharges, and documentation review for billing.
- Supervise & coach nursing/caregiving staff: perform competency assessments, lead caregiver‑client introductions, facilitate orientation & in‑service programs, be available for urgent clinical issues.
- Current, unrestricted RN license in Ohio.
- Degree in Nursing from a National League for Nursing‑accredited program.
- Minimum 3 years clinical experience, including at least 1 year case management.
- Preferred: 1 year in a home health agency & healthcare management.
- Preferred: 1 year working with pediatric clients.
- Strong nursing assessment skills, attention to detail.
- Ability to supervise, educate, and direct staff.
- Thorough understanding of state & federal regulatory compliance.
- Familiarity with Joint Commission policies & survey preparedness.
- Adherence to HIPAA & client confidentiality.
- Ability to manage complaints and complex situations with sound clinical judgment.
- Experience in client teaching & public speaking.
- Excellent customer service & tactful communication with clients, staff, partners.
- Active listening, empathy, leadership, motivation.
- Can multi‑task, troubleshoot, remain composed under pressure.
- Comfortable working independently or within a team.
- Technologically proficient (Microsoft Office, EMR).
- Flexible schedule; on‑call coverage as needed within territory and surrounding region.
- Valid driver’s license, reliable transportation, current auto insurance.
- Willingness to travel daily—including weekends, evenings, holidays.
- Current CPR certification; negative TB test within past 12 months.
- Up‑to‑date vaccinations (Influenza, COVID‑19, Tetanus, Diphtheria, Pertussis, Rubella, Varicella, MMR, Hepatitis B).
- Ability to pass drug screening, background check, and criminal record clearance.
- Physically able to stand, walk, squat, bend, kneel, reach, twist, push & pull; lift up to 50 pounds.
- Competitive pay rate: $35‑40 per hour.
