Position: Full-Time License Practical Nurse (LPN) - Home Home Health - $35-40

Full-Time Licensed Practical Nurse (LPN) – Home Health – $35-40/hr

We are a small business with a big personality, seeking a dedicated LPN who is flexible, hard‑working, creative problem solver, and willing to roll up their sleeves to care for our clients.

Job Responsibilities Rapid Response & Readiness
  • Flexible 40+ work hours during Monday‑Friday 8:00‑20:00, some weekend shifts; occasional overnight shifts if required.
  • Be ready to leave for a shift within 30 minutes during on‑call hours.
  • Maintain readiness: keep uniform & , childcare arranged, cell phone charged, transportation available, equipment & supplies accessible.
  • Support office operations when not on field shifts.
Direct Client Care & Case Management
  • Conduct initial assessments, re‑assessments, and safety evaluations.
  • Deliver skilled nursing per Ohio Nurse Practice Act (infusions, wound care, injections, etc.).
  • Observe and report changes in client condition, documenting thoroughly.
  • Follow infection control protocols and universal precautions.
  • Respond in emergencies, using equipment safely.
Administrative Support
  • Develop & maintain person‑centered Plans of Care; obtain physician signatures.
  • Coordinate care with families, providers, internal staff.
  • Assist with referral intake & follow‑up communications.
  • Assist Clinical Director with weekly chart reviews for compliance, incident reporting, timely discharges, and documentation review for billing.
  • Supervise & coach nursing/caregiving staff: perform competency assessments, lead caregiver‑client introductions, facilitate orientation & in‑service programs, be available for urgent clinical issues.
Job Requirements Licensure & Education
  • Current, unrestricted RN license in Ohio.
  • Degree in Nursing from a National League for Nursing‑accredited program.
  • Minimum 3 years clinical experience, including at least 1 year case management.
  • Preferred: 1 year in a home health agency & healthcare management.
  • Preferred: 1 year working with pediatric clients.
Clinical & Regulatory Competencies
  • Strong nursing assessment skills, attention to detail.
  • Ability to supervise, educate, and direct staff.
  • Thorough understanding of state & federal regulatory compliance.
  • Familiarity with Joint Commission policies & survey preparedness.
  • Adherence to HIPAA & client confidentiality.
  • Ability to manage complaints and complex situations with sound clinical judgment.
  • Experience in client teaching & public speaking.
Interpersonal & Professional Skills
  • Excellent customer service & tactful communication with clients, staff, partners.
  • Active listening, empathy, leadership, motivation.
  • Can multi‑task, troubleshoot, remain composed under pressure.
  • Comfortable working independently or within a team.
  • Technologically proficient (Microsoft Office, EMR).
Availability & Health Requirements
  • Flexible schedule; on‑call coverage as needed within territory and surrounding region.
  • Valid driver’s license, reliable transportation, current auto insurance.
  • Willingness to travel daily—including weekends, evenings, holidays.
  • Current CPR certification; negative TB test within past 12 months.
  • Up‑to‑date vaccinations (Influenza, COVID‑19, Tetanus, Diphtheria, Pertussis, Rubella, Varicella, MMR, Hepatitis B).
  • Ability to pass drug screening, background check, and criminal record clearance.
  • Physically able to stand, walk, squat, bend, kneel, reach, twist, push & pull; lift up to 50 pounds.
