During visits lasting up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps improve quality of care and allows us to potentially close multiple care gaps with a single visit.

• Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, referrals, follow-up, and documentation according to Matrix guidelines and protocols.
• Work collaboratively with physicians, case managers, social workers, family members, caregivers, and other medical personnel as appropriate.
• Coordinate with Primary Care Physician (PCP) on patient education and follow-up care.
• Provide services in various settings, including home visits and skilled facility visits.

Type: Full Time Permanent

Location: Home, SNF, and other community settings within Massachusetts and bordering states, based on business needs.

Hours: Monday – Friday, 8:00 AM - 5:30 PM (hours may vary, full-time expected)

Salary: $124,800 - $151,800 per year, based on experience

Required:

• Master’s Degree or equivalent experience with NP licensure
• Current RN and NP licensure in the practice state, including prescriptive authority or ability to obtain it
• Board certification by AANP, ANCC, or AACCN in an approved specialty
• Current BLS, ACLS, or CPR certification
• At least 3 years of Nurse Practitioner experience
• Strong computer skills and familiarity with medical record software
• Excellent communication skills
• Flexibility to adapt to change

• Travel to meet patients where they live within the state and possibly neighboring states.
• Willingness to travel up to 80-90% of the time, including overnight stays and weekends.
• Valid driver’s license, ability to drive, and proof of adequate auto insurance.
