About Us (Organization focus):
We are dedicated to providing exceptional care to patients with congestive heart failure (CHF). We are seeking a skilled and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to help reduce hospitalizations and improve quality of life for patients through advanced CHF management and GDMT (Goal Directed Medical Therapy).
- Patient Assessment: Conduct comprehensive assessments to identify early signs of CHF exacerbation, enabling timely interventions to prevent hospitalizations.
- Treatment Planning: Develop and implement personalized treatment plans aimed at achieving maximal doses of GDMT, optimizing heart function, and reducing symptoms.
- Medication Management: Titrate medications to target GDMT doses, monitor side effects, and ensure patient adherence.
- Patient Education: Educate patients and families on CHF management, medication adherence, lifestyle changes, and recognizing early symptoms of worsening heart failure.
- Symptom Management: Proactively manage symptoms through home-based interventions, such as diuretic adjustments and dietary modifications, to prevent acute exacerbations.
- Coordination of Care: Collaborate with cardiologists, home health nurses, and social workers to provide comprehensive, coordinated care in the home setting.
- Follow-Up Care: Conduct regular home visits or telehealth consultations to monitor progress, adjust treatment plans, and provide ongoing support.
- Emergency Care: Provide immediate care for acute CHF symptoms at home, reducing the need for emergency room visits and hospital admissions.
- Patient Advocacy: Ensure patients have access to resources such as home health services and support groups to enhance quality of life.
- Research and Quality Improvement: Participate in projects focused on home-based CHF management and the effectiveness of GDMT in reducing hospitalizations and improving outcomes.
- Current NP license or GA license in the state
- Certification in cardiology or CHF management is ideal but not required
- Experience in managing CHF patients, particularly in a home-based setting, is ideal but not required
- Strong knowledge of GDMT and CHF management guidelines
- Excellent communication and patient education skills
- Ability to work collaboratively within a multidisciplinary team
