• Travel to hospitals and rehab facilities to build professional partnerships
• Care for our vulnerable population of seniors and adults with disabilities, using the right PPE
• Communicate clearly with people at all levels, both verbally and in writing
• Observe and report any signs or symptoms indicative of changes in a client’s condition
• Provide best‑in‑class care management to clients
• Coach and train new caregivers, serving as a role model for excellence

• Registered Nurse (RN) with current licensure to practice professional nursing in the state
• 2 years of nursing experience (home health or hospice preferred)
• Current CPR certification
• Knowledge and ability to perform nursing procedures under the direction of a physician as outlined in the State’s Nurse Practice Act
• Compliance with requirements for employees providing direct client care
• Valid driver’s license and insured automobile for work or adequate transportation access
• Availability to work on weekends and holidays as required
• English proficiency (read, write, speak, and understand)
• Participation in in‑service education programs as mandated by state law

• Primarily works in client’s place of residence; travel may be required
• Shift times and days vary with client needs
• Nurse Liaisons will go into rehabs and hospitals to build professional partnerships
