Primary Care Telephone Triage Nurse (Hybrid) Starting Pay: $34.00 – $39.25 per hour (based on experience) $5,000 Sign-On Bonus. 3 Days Onsite at Goodwin Community Health (Somersworth, NH) 2 Days Remote Serving Both Goodwin Community Health & Families First Health & Support Center.

• Serve as the first point of clinical contact via phone and patient portal.
• Conduct thorough telephone assessments using evidence‑based triage protocols.
• Prioritize and direct patients to the appropriate level of care.
• Provide clinical advice, education, and emotional reassurance.
• Collaborate with providers, nurses, and multidisciplinary teams.
• Recognize and elevate urgent or emergent situations.
• Document accurately and promptly in the EHR.
• Participate in quality improvement and ongoing training.

• 3 days in‑person at Goodwin Community Health (Somersworth, NH).
• 2 days remote from home.
• Collaborate with both GSCH primary care sites (Somersworth & Portsmouth).

• Make a real impact through skilled assessment and communication.
• Use your RN expertise in a focused, patient‑centered environment.

• Registered Nurse (RN) – REQUIRED
• Experience in primary care, ambulatory care, or telephone triage preferred.
• Current BLS certification.
• Excellent clinical judgment and communication skills.
• Ability to work both independently and as part of a team.
• Comfort with EHR systems and patient portal communication.

• $5,000 Sign‑On Bonus.
