Position Overview
Registered Nurse (RN)/Case Manager at Advanced Maintenance Solutions, LLC.
Reports to Director of Nursing.
Base Pay Range$60.00/hr - $100.00/hr
Pay: $60.00 - $100 per visit PRN rates, mileage reimbursement, iPad with company paid cellular data, opportunities for full-time positions with benefits.
Job DescriptionThe registered nurse plans, organizes, and directs home care services emphasizing health education and experience. The RN Case Manager travels to a patient’s home to administer services dictated by their physician and At Home Quality Care while helping patients maintain their independence and quality of life.
Essential Job Functions/Responsibilities- Assess and chart observations of the patient’s condition and needs at each home visit.
- Complete evaluation tasks, including medication reviews and vital signs.
- Initiate and revise the Plan of Care as patient status and needs change.
- Provide education to patients and family members on proper home health procedures and strategies.
- Develop a care plan based on nursing diagnosis and incorporate therapeutic, preventive and rehabilitative nursing actions.
- Administer medications and treatments as prescribed by the physician.
- Provide health care instructions to the patient per assessment and plan of care.
- Identify discharge planning needs, prepare patient and family for discharge.
- Act as Case Manager when assigned by Clinical Supervisor and coordinate care for assigned caseload.
- Coordinate with physicians, physical therapists, occupational therapists, MSWs, and other team members. Perform a supervisory visit with CNA (if part of the care plan) once every two weeks.
- Prepare clinical notes and update primary physician as needed.
- Communicate with physician and office regarding patient needs and report changes; obtain physician orders.
- Communicate with community health-related persons to coordinate care plan.
- Report updates, concerns, and issues to home office.
- Graduate of an accredited school of nursing.
- Current licensure in Illinois and current CPR certification.
- Management experience not required; responsible for supervising home health aides.
- Excellent observation, verbal and written communication skills, problem-solving and basic math skills.
- Ability to walk, stand, lift, position, transfer patients, and handle supplies; good visual acuity and hearing.
- Licensed driver with automobile insured in IL; automobile in good working order.
- Experience with Axxess highly desired.
Flexible scheduling; choose days, time, location, full-time or part-time, and number of cases.
