Utilize your clinical skills to create, implement, and execute comprehensive plans of care that promote wellness and prevent rehospitalization for patients in their homes. You will oversee and teach patients, families, and caregivers on wound care, blood draws, tube feedings, tracheostomy care, Foley catheters, CHF management, and medication education.

• Perform a full assessment of the patient & family upon initial visit.
• Use assessment data to develop an advanced plan of care including rehabilitation.
• Work one‑on‑one with patients and families to achieve improvement or stability.
• Case‑manage the patient’s care episode with an interdisciplinary clinical team.
• Prepare clinical notes, orders, and charting within a compliant timeframe.
• Collaborate with physicians and agency staff to deliver the best care possible.

• Must be willing to work either Saturday or Sunday.
• Current and unrestricted California RN license.
• Current BLS/CPR certification.
• Valid California driver’s license.
• High level of clinical knowledge in home health care (preferred).
• Experience with high‑acuity patients (plus).
• Experience working with an interdisciplinary team (desired).
• Case‑management fundamentals (desired).
• Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, organized.
• Computer literacy assessment required before in‑person interview.
