Overview

We are currently seeking an experienced Registered Nurse (RN) for a Travel Contract Hospice Home Health job in Milwaukee, Wisconsin. • Provide skilled nursing care to hospice patients in their homes.
• Perform patient assessments, symptom management, and care planning.
• Administer medications and treatments per physician orders.
• Educate patients and families on disease progression, comfort measures, and care plans.
• Coordinate care with physicians, hospice team members, and caregivers.
• Maintain accurate and timely clinical documentation.

• Active RN license (Wisconsin or compact accepted).
• Hospice and/or Home Health nursing experience preferred.
• BLS certification required.
• Strong communication, time management, and organizational skills.
• Ability to work independently in a home-based environment.
