Monogram Health is looking for skilled Nurse Practitioners and Physician Assistants eager to make a meaningful difference in patients’ lives. The Advanced Practitioner is a key member of an integrated Care Team that includes a Registered Nurse and a Social Worker. Our patients often face multiple serious diseases; our NPs and PAs help improve quality of life at home and slow the progression of kidney disease, enabling positive outcomes.
Key Responsibilities- Conduct comprehensive annual wellness exams in patients’ homes and virtually.
- Counsel and educate patients and families about benefits and programs to promote healthier lifestyles.
- Document clinical information accurately: chief complaint, diagnoses, medical, family, and social history, review of systems, exams, medications, allergies, assessment, and plan.
- Coordinate care with primary care providers, specialists, and ancillary services.
- Complete all documentation and paperwork in a timely manner.
- Maintain quality of care standards as defined by the practice.
- Operate a remote, state‑based model; attend periodic training or meetings outside the state.
- Deliver evidence‑based, timely care that reduces avoidable hospitalizations and maximizes quality of life.
- Prescribe medications, order tests, and collaborate with Monogram physicians.
- Measure outcomes daily to improve patient quality results.
- Educate patients on preventive monitoring and facilitate scheduling of diagnostic testing.
- Assist patients in enrolling for access to educational videos.
- Participate in integrated care team meetings.
- Apply disease diagnosis and prevention knowledge throughout practice.
- Create and implement treatment plans tailored to patients’ health status.
- Follow up and evaluate patient status as needed.
- Other duties as assigned.
- Bilingual (English/Spanish) highly preferred.
- BLS certification required; the company supports completion through onboarding.
- Active, unrestricted RN and NP or PA license.
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
- Current, unrestricted DEA certificate.
- Ability to work autonomously and without direct supervision.
- Valid driver’s license and car insurance with access to reliable transportation.
- Proficiency with medical instruments and equipment necessary for the role.
- Experience with computer‑based data management programs, electronic medical records, and point‑of‑care technology.
- Excellent verbal and written communication skills with patients, families, physicians, and community partners.
- Sound understanding of federal and state regulations, including HIPAA and OSHA.
- Minimum of 2 years direct patient care experience.
- Managed Care/IPA/Health Plan experience.
- Experience conducting annual wellness visits virtually or in the home.
