Position Title: Part-Time RN Case Manager
Hours: 25 hours/week
Position SummaryWe are seeking a dedicated and organized Registered Nurse (RN) to serve as a Part-Time Care Coordinator. This hybrid position (on-site and remote) will focus on managing the ongoing care of assigned members to ensure timely, accurate, and compliant care coordination.
Key Responsibilities- Complete and maintain member Care Plans on a monthly schedule.
- Participate in Interdisciplinary Team (IDT) meetings to review and update member care.
- Communicate with hospitals and other clinical providers to obtain updates on member status.
- Accurately document clinical information in the EMR in accordance with agency policies.
- Collaborate with internal staff and external providers to ensure comprehensive, person-centered care.
- Attend Team clinical meetings.
- Current RN license in Texas.
- Strong organizational and time management skills.
- Proficiency with EMR/charting systems.
- Excellent communication skills, both written and verbal.
- Ability to work independently in a hybrid setting.
- Hybrid position with a combination of on-site and remote work.
- 25 hours per week, flexible schedule.
