Locums; Potential Perm Hybrid Nurse Practitioner or Physician Assistant

We are seeking an experienced

Nurse

Practitioner (NP) or Physician Assistant (PA) for aLocums assignment with potential to convert to Permat a primary care practice servinggeriatric patients in avalue-based care model. This is ahybrid rolewithapproximately
40% coverage in our outpatient clinic and 60% home-basedpatient care.

Compensation:$75 – $85 per hour

Position Details:

• Schedule:Monday – Friday (2–5 days perweek; flexible days that suit your availability)
• Patient Load:
• Home Care: 3–5 patients per day
• Clinic:
  Small census of 7–10 patients perday
• Setting:Hybrid (Outpatient clinic +home-based care), value-based primary care for elderly patients

Responsibilities:

• Deliver comprehensive primary care services,including preventive care, chronic disease management, and acute care
• Diagnose, treat, prescribe medications, and develop individualized treatment plans
• Order and interpret diagnostic tests (labs,imaging, etc.)
• Document accurately in the EHR system
• Provide patient education and promote wellness initiatives
• Collaborate with physicians, nurses, and support staff to ensure coordinated, high-quality care
• Refer patients to specialists and managefollow-ups
• Ensure compliance with state, federal, and professional regulations

Requirements:

• Active NP or PA license in NY
• Minimum 2 years of primary care experience
• National Board Certification
• Bilingual a plus
