DaVita Integrated Kidney Care (IKC) is looking for a full‑time Registered Nurse Case Manager in Fairfield, CA. The RN Case Manager supports complex patients with CKD/ESKD, coordinating care transitions, managing social determinants, and collaborating with the nephrology team to deliver cost‑effective, high‑quality care.

• Build empathetic relationships with patients and families.
• Coordinate care transitions and manage ongoing needs for complex/at‑risk patients.
• Identify and address social determinants of health, including medical, emotional, educational, community, and financial concerns.
• Streamline and enable effective transition of care when needed.
• Assess patients to identify unreported medical conditions or changes that may lead to adverse outcomes.

• Current RN license required.
• BSN preferred.
• Minimum of 2 years RN experience with healthcare performance coaching.
• Current CPR or BLS certification.
• Valid driver’s license and willingness to travel within assigned territory.
• Intermediate computer skills; proficient in MS Word, Excel & Outlook.
• Home office with minimum 1 MB upload and 1 MB download internet speed.

• 2+ years case or chronic care management experience.
• 1+ year utilization management experience.
• Spanish speaking preferred in some markets.
