Job Summary
The Homecare Admission Registered Nurse plays a vital role in facilitating the admission of new patients into the agency’s skilled home health, hospice, and palliative care programs. This position ensures the safe and high-quality delivery of nursing and home health services while adhering to best practice standards. As part of the admission process, the Admission RN is responsible for the accurate and efficient completion of all required documentation, including OASIS, HIS, comprehensive assessments, and the development of an initial plan of care.
The Admission RN is expected to complete a minimum of two admissions per day (or the equivalent of six visit points). Based on patient and agency needs, the nurse may also be required to perform follow-up visits. Additional compensation is provided for visits exceeding 24 visit points in a 32-hour workweek or 30 visit points in a 40-hour workweek, per the agency’s standard per-visit rates.
This role reports directly to the Homecare Clinical Manager.
Job Responsibilities
- Provide skilled nursing care, including patient admission and discharge, in a home-based, family-centered environment while maintaining complete and accurate patient records in electronic and/or paper formats.
- Educate, demonstrate, supervise, and assess care provided by peers, Home Health Aides (HHAs), family members, and caregivers, fostering a therapeutic relationship that promotes self-advocacy and independence.
- Ensure timely and accurate documentation of patient information, including OASIS and HIS data sets, medication reconciliation, service orders, time sheets, and other required patient and employment records.
- Conduct systematic assessments of patients' physical, psychosocial, emotional, cultural, spiritual, and discharge needs, considering their age and developmental level (patient population: 14 years and older to geriatric).
- Develop and implement individualized plans of care tailored to each patient’s needs and conditions.
- Utilize effective verbal and written communication to ensure comprehensive documentation and safe patient hand-offs.
Education and Experience Requirements
- Current license to perform as a registered nurse in the State of Connecticut required.
- Valid Connecticut Driver’s License and proof of current insurance coverage required.
- Graduate from an accredited School of Nursing required.
- Two years of recent medical/acute care or critical care experience as a registered nurse, or an equivalent level of experience preferred. Achieve Home Health Care specific competencies within the timeframe determined by the department.
- Proficiency in reasoning, mathematics, reading, writing, and speaking with confidence, poise, and voice control using correct English and a well-modulated tone.
- Strong skills in coordination, supervision, analysis, instruction, customer service, and precision work. Must effectively utilize tools, materials, and work aids, exercising sound judgment in selecting and adjusting them to meet required standards.
