Agency: Genesis Home Care
Schedule: Monday - Friday, 8:00AM - 5:00PM with on-call requirements
Territory: Clinicians will work within Chesterhill, McConnelsville, Malta, Stockport, Bremen, Rushville, Corning, Hemlock, Junction City, New Lexington, New Straitsville, Shawnee, Moxahala, and Glouster
How You'll Make a Difference
Care and compassion are at the heart of what you'll do as a Licensed Practical Nurse (LPN). You'll provide home-based nursing care for patients as directed by the physician, following order sets and goals created by the Case Manager. You'll interact with patients' families and be part of their recovery journey, gaining real-world perspective on patient care.

How You'll Work
You will provide skilled nursing care in patients' homes, working autonomously and managing your time efficiently within your scope of practice.
Major Responsibilities
- Provide skilled nursing care as ordered, monitor patient conditions, and notify the case manager of changes.
- Administer medications, educate patients on preventative measures, and help reduce re-hospitalizations.
- Educate patients and families on disease processes using teach-back methods.
- Complete all clinical documentation following protocols and guidelines.
- Follow agency policies, attend training, and communicate schedule changes.
Skills & Requirements
- Current LPN license in the state of employment.
- At least one year of experience as an LPN, preferably in home care.
- Valid driver’s license and auto insurance.
- Ability to meet physical demands.
