Job Title
Registered Nurse (RN) / Case Manager
Reports To: Director of Nursing
LocationProspect Heights, IL
Salary & BenefitsBase Pay Range: $60.00/hr – $100.00/hr
Pay Structure: $60.00 – $100.00 per visit (PRN rates) with mileage reimbursement, company‑provided iPad with cellular data, and opportunities for full‑time positions with benefits.
Job OverviewThe Registered Nurse (RN) / Case Manager plans, organizes, and directs home care services emphasizing health education and patient independence. Travel to patient homes to administer physician‑directed services and support patients’ quality of life.
Key Responsibilities Patient Care- Assess and chart observations of the patient’s condition and needs at each home visit.
- Complete evaluation tasks, including medication reviews and vital signs.
- Initiate and revise the Plan of Care as patient status and needs change.
- Provide education to patients and family members on proper home health procedures and strategies.
- Develop a care plan based on nursing diagnosis, incorporating therapeutic, preventive, and rehabilitative nursing actions.
- Administer medications and treatments as prescribed by the physician.
- Provide health‑care instructions to the patient as appropriate per assessment and plan of care.
- Identify discharge planning needs; prepare patient and family for discharge.
- Act as Case Manager when assigned by the Clinical Supervisor, coordinating patient care for the caseload.
- Coordinate with physicians, physical therapists, occupational therapists, MSWs, and other care‑team members; perform supervisory visits with CNAs every two weeks (when part of the plan).
- Prepare clinical notes and update the primary physician when necessary.
- Communicate with physicians and office staff regarding patient needs and report changes in condition; obtain or receive physician orders as required.
- Communicate with community health‑related persons to coordinate the care plan.
- Report with home office on updates, concerns, and issues regarding the patient.
- Graduate of an accredited school of nursing.
- Current licensure in the state of Illinois and current CPR certification.
- Experience supervising home‑health aides is not required.
- Excellent observation, verbal and written communication, problem‑solving, math, and nursing skills.
- Physically capable of prolonged walking or standing; able to lift, position, or transfer patients and supplies; able to reach, stoop, bend, kneel, or crouch as required.
- Must be a licensed driver with an automobile insured under the state of Illinois; automobile must be in good working order.
- Experience with Axxess is highly desired.
