Weekend Licensed Practical Nurse (LPN), $10,000 Bonus!
Alternate Solutions Health Network is looking for a Weekend Licensed Practical Nurse (LPN) to provide home-based nursing care on Tuesday, Wednesday, and Saturday from 8:30 am to 5:00 pm. The position includes a guaranteed base hourly rate, uncapped bonus structure, mileage reimbursement, and comprehensive benefits.

How You’ll Make a DifferenceAs an LPN you will work autonomously, providing skilled nursing care in patients’ homes as directed by the attending physician and following orders from the Case Manager. You will interact with patients and families, educating them about disease processes and encouraging recovery.

You will be responsible for all duties within the scope of practice outlined by state regulations and for maintaining accurate clinical documentation.
Major Areas of Responsibility- Plan of Care – Provide skilled nursing care as ordered, monitor patient conditions, and notify the case manager of any changes.
- Minimize Patient Risk – Administer medication, educate patients on preventive measures, and reduce re‑hospitalizations.
- Family Educator/Advocate – Use teach‑back methods to ensure patient and family understanding.
- Policy & Documentation – Complete clinical documentation per agency protocol and Medicare/Federal guidelines.
- Compliance – Follow agency policies, procedures, and regulations, and communicate schedule changes to supervisors.
- Operations – Attend in‑service training and mandatory agency meetings.
- Compassionate communicator with a positive attitude.
- Patience when working with patients, families, physicians, and coworkers.
- Attention to detail and observant, following directions.
- Licensed Practical Nurse (LPN) with a current license in the state of employment.
- Minimum one‑year experience as an LPN in an acute care setting (home‑care experience preferred).
- Valid driver’s license and auto insurance in your name as a driver.
- Capable of meeting all physical demands of the position.
