BriefJob Description
The registered nurse plans, organizes, and directs home care services emphasizing health education and experience. The RN Case Manager travels to patients’ homes to administer services dictated by their physician and At Home Quality Care, while helping patients maintain independence and quality of life.
Essential Job Functions / Responsibilities- Assess and chart observations of the patient's condition and needs at each home visit.
- Complete evaluation tasks, including medication reviews and vital signs.
- Initiate the Plan of Care and make necessary revisions as patient status and needs change.
- Provide education to patients and family members on proper home health procedures and strategies.
- Develop a care plan based on nursing diagnosis and incorporate therapeutic, preventive, and rehabilitative nursing actions.
- Administer medications and treatments as prescribed by the physician.
- Provide health care instructions to the patient as appropriate per assessment and plan of care.
- Identify discharge planning needs as part of the care plan. Prepare patient and family for discharge.
- Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for assigned caseload.
- Coordinate with physicians, physical therapists, occupational therapists, MSWs, and other individuals in the patient care plan. Perform a supervisory visit with a CNA (when part of the care plan) once every two weeks.
- Prepare clinical notes and update the primary physician when necessary.
- Communicate with the physician and office regarding the patient's needs and report any changes in the patient's condition; obtain or receive physician's orders as required.
- Communicate with community health‑related persons to coordinate the care plan.
- Report with home office any updates, concerns, and issues regarding the patient.
- Graduate of an accredited school of nursing.
- Current licensure in the state of Illinois and current CPR certification.
- Management experience not required. Responsible for supervising home health aides.
- Excellent observation, verbal and written communication skills, problem‑solving skills, basic math skills, nursing skills per competency checklist.
- Must be able to walk or stand for prolonged periods, lift, position, transfer patients and supplies, and perform reaching, stooping, bending, kneeling, or crouching. Adequate visual acuity and hearing to perform required nursing skills.
- Must be a licensed driver with a vehicle insured under the state of IL and in good working order.
- Axxess experience is highly desired.
