• Provide care coordination services, including disease management and case management
• Educate, motivate, and empower members to manage their health conditions
• Assist with the management of member health plan benefits and alternative services
• Work to improve access and quality of care for members with acute and complex healthcare needs
• Collaborate with physicians in the management of patient care

• 5 years of clinical nursing experience
• Current unencumbered Registered Nurse License in the state of residency upon hire
• Experience working collaboratively with physicians in patient care management

• Bachelor s Degree in Nursing or health education
• Current nursing experience in cardiology, endocrinology, pediatrics, obstetrics, oncology, respiratory, or health education
