RN - Hospice

Overview

RN - Hospice field position serving the Charlotte metro area and Harrisburg (western corridor of Cabarrus County). Remote work to start; initial day at home then field visits to patient homes and long-term care facilities. Office presence for biweekly IDG meetings, orientation, training and to obtain supplies.

Facility: 5003 Hospice Lane, Kannapolis, NC 28081

Hours of Operation: 08:30 to 17:00, Monday–Friday. Availability during those hours with potential to pick up night/weekend shifts if desired.

• Serve End of Life and Hospice eligible patients in the home setting and in LTC facilities, including Assisted Living and SNFs.
• Travel to patient homes; this is a field position requiring travel and autonomy in scheduling.
• Biweekly Case Conference (IDG) every other Wednesday; document ongoing eligibility and hospice criteria.
• Manage visits and documentation using Homecare Homebase EMR; mileage is reimbursed per IRS rate and tracked via mapping/software for payroll.
• Typical caseload: average 14–15 patients; goal of 5–6 visits per day, understanding that daily numbers may vary; territory is assigned by location and may fluctuate with patient load.
• Autonomy to manage own schedule; potential for additional visits as needed.

• Registered Nurse (RN) with 1–2 years of hospice experience in the home setting; home visits required.
• Current BLS certification; valid Driver's License and proof of vehicle insurance.
• Experience with Homecare Homebase EMR is required.
• Experience in Hospice/Home Health; familiarity with visiting patients in Long Term Care settings.
• Clinical skills including catheter care, feeding support (g-tubes), CAD pumps for pain control, medication management, psychosocial support; strong communication and assessment of patient/family needs; bereavement support and education during EOL care.

• Interdisciplinary team includes Peer Hospice RNs, Nurse Educator, Clinical Supervisors (after-hours support), Social Workers, Case Manager (office-based), Medical Director, Nurse Practitioner (symptom management calls), Chaplain, Bereavement Counselor, Home Health Aide.
• Technology/Equipment:
  Homecare Homebase EMR; tablet and laptop provided (laptop used for home charting).

• Orientation at least 1 week (shadowing/training with other caregivers), longer if needed based on skill level.
