Nurse Practitioner – Advanced Practice Provider

Monogram Health is seeking experienced Nurse Practitioners and Physician Assistants to join an integrated Care Team that delivers in‑home, evidence‑based care for patients with complex chronic conditions.

Roles & Responsibilities
  • Conduct comprehensive annual wellness exams for patients both in their homes and virtually.
  • Provide education and counseling to patients and families about disease management, preventive care, and available health programs.
  • Document patient encounters, including chief complaint, history, review of systems, examination, assessment, and plan in the electronic medical record.
  • Coordinate care with primary providers, specialists, and ancillary services to ensure seamless patient transition.
  • Maintain high quality of care standards and accurate, timely documentation.
  • Deliver evidence‑based, timely care that reduces avoidable hospitalizations and maximizes patient quality of life and satisfaction.
  • Prescribe medications, order laboratory and diagnostic tests, and collaborate with the Monogram physician.
  • Participate in integrated care team meetings and continuous quality improvement activities.
  • Travel to patient homes or remote offices as required; attend periodic training and meetings outside state.
  • Other duties as assigned.
Position Requirements
  • Bilingual (English/Spanish) highly preferred.
  • Active and unrestricted NP or PA license, board certified (ANCC/AANP or NCCPA).
  • DEA certificate, current and unrestricted.
  • Ability to practice autonomously without direct supervision; strong problem‑solving and clinical judgment.
  • Valid driver’s license, reliable transportation, and car insurance.
  • Proficiency with medical equipment, computer‑based data management, EMR, and medical records technology.
  • Excellent verbal and written communication with patients, families, and healthcare partners.
  • Sound understanding of HIPAA, OSHA, and state/federal regulations.
  • Minimum 2 years of direct patient care; experience conducting annual wellness visits (virtual or in‑home) preferred.
  • Managed care / IPA / Health Plan experience a plus.
