Join Providence as a Clinical Program Coordinator RN and play a pivotal role in providing care coordination services to Providence Health Plan members.

Your primary responsibility will be to deliver comprehensive care coordination services, including disease management education, case management triage and referrals, transition of care planning, end‑of‑life care planning, and acute/catastrophic case management. You will assist members with complex health care needs and chronic conditions, help families manage day‑to‑day functional needs, and guide members on navigating health plan benefits. This position operates within the health plan framework, balancing the management of medical expenses with improving access and quality of care.

• 5 years of clinical nursing experience.
• Current, unencumbered Registered Nurse license in your state of residency.
• Experience collaborating with physicians to manage patient care.

• Bachelor’s degree in Nursing or Health Education.
• Clinical experience in cardiology, endocrinology, pediatrics, obstetrics, oncology, respiratory, or health education.

• Oregon – Non‑Portland: $45.90–$71.40 per hour
• Oregon – Portland: $49.34–$76.59 per hour
• Washington – Western (except Tukwila): $51.43–$79.84 per hour
• Washington – Southwest (Olympia, Centralia & below): $49.34–$76.59 per hour
• Washington – Tukwila: $51.43–$79.84 per hour
• Washington – Eastern: $43.90–$68.16 per hour
