Nurse Practitioner - Hybrid

Our client seeks a Nurse Practitioner to join their growing team around Elk Grove, CA.

The organization partners with nephrologists throughout California. Nurse Practitioners provide hands-on case management and clinical education to prevent hospitalizations and the need for dialysis.

Base pay range: $145,000/yr - $160,000/yr. Final pay is based on skills and experience.

• Travel to local nephrology clinics 3-4 days per week.
• Perform administrative work and provide telehealth services from home 1-2 days per week.
• Perform comprehensive physical evaluations.
• Develop individualized care plans and ensure appropriate follow up through the care continuum.
• Work collaboratively with an interdisciplinary team including nephrologists, RNs, dietitians, cardiologists, etc.
• Provide coordinated care services between the patient, family, physicians and other care team members.

• Master’s Degree in Nursing
• Licensed Nurse Practitioner in the state of California
• 1+ year of experience as a Nurse Practitioner within Nephrology, Cardiology, Primary Care or Internal Medicine
• Current DEA or eligible to obtain within 90 days of hire
• Current driver’s license and insurance within California
