Hospice RN Case Manager – Lafayette

Our team is growing. We are seeking a RN to serve in the Lafayette and Crawfordsville area.

Job status: Permanent

• The Registered Nurse (RN) is responsible for the delivery of patient care through nursing process of assessment, planning, implementation, and evaluation. The RN plans, organizes and directs hospice care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
• Direct and coordinate hospice care services, utilizing nursing knowledge to provide home-based patient care.
• Provide community health education/experience to patients and families.
• Follow regulatory requirements and organization policies to ensure safe, compliant care.

• Valid Indiana Nurse License (RN).
• Experience in hospice or long-term care is preferred.
• Graduate of an accredited school of nursing.
• Excellent communication and assessment skills; ability to work independently in home settings.
