Visiting Registered Nurse (RN) – Advanced Maintenance Solutions, LLC.
Base pay: $50.00/hr.
Pay: $50 per regular visit, $100 per start of care or resumption of care, $65 per recertification or discharge.

The Registered Nurse (RN)/Case Manager plans, organizes, and directs home care services with emphasis on health education and patient independence. Visits are performed in patients’ homes and include administration of medication, education, and coordination with other healthcare providers.
Essential Job Functions / Responsibilities- Assess and chart patient conditions and needs at each visit.
- Complete evaluation tasks, including medication reviews and vital signs.
- Initiate and revise the Plan of Care based on patient status.
- Provide patient and family education on home health procedures and strategies.
- Develop a care plan establishing goals based on nursing diagnosis.
- Administer medications and treatments as prescribed.
- Give health care instructions per assessment and plan of care.
- Identify discharge planning needs and prepare patient and family for discharge.
- Act as Case Manager when assigned, coordinating care for assigned caseload.
- Coordinate with physicians, physical therapists, occupational therapists, MSWs and other team members.
- Perform a supervisory visit with CNA weekly or biweekly as required.
- Prepare clinical notes and update primary physician as necessary.
- Communicate with physicians and office regarding patient needs and orders.
- Coordinate with community health personnel to support care plan.
- Report updates, concerns and issues to home office.
- Graduate of an accredited nursing school.
- Current licensure in Texas and current CPR certification.
- Proficiency in supervising home health aides (management experience not required). li>Excellent observation, verbal and written communication skills, problem‑solving and basic math abilities.
- Physical ability to walk, stand, lift, transfer patients and perform required nursing tasks.
- Licensed driver with an active vehicle insured in Texas; vehicle in good working condition.
