Northwest Kidney Centers seeks a full-time day shift Home Program Nurse Care Manager (Home Dialysis Training Nurse) to support Renton, Burien and Panther Lake Kidney Centers in peritoneal dialysis and hemodialysis.
PurposeThe Home Program Nurse Care Manager (Home Dialysis Training Nurse) is accountable for the delivery and documentation of care for patients dialyzing at Northwest Kidney Centers and in their home. The Care Manager uses a collaborative practice model focusing on outcomes to enhance the quality of patient care through effective and efficient management and monitoring of patient care programs. This includes assessments, planning care, directing, and evaluating patients and their home environment and clinical results, education and training as well as services and programs that enhance patient care.
ShiftFull-time, day shift (default , but can be 4x10s or 5x8s), Mon-Fri.
Required qualifications- Equivalent of Associate or Baccalaureate degree with nursing major
- Current registered nursing license in the state of Washington
- One year of previous nursing experience.
- Previous dialysis experience
