Job Summary

Creates an environment that leads to a positive patient care experience by facilitating problem solving and decision making. Ensures accuracy and timeliness on all functions related to insurance prior authorization, plan benefits, and medical necessity documentation. Serves as a liaison between patients, physicians and all other participants of the health care team.

Responsibilities
  • Meet with patients/families in person to discuss continuum of care services.
  • Meet/communicate with referral sources (case management, social work, clinics, providers, SNF/ALF facilities, etc).
  • Coordinate care between continuum of care departments, such as branch departments and business development.
  • Eligibility/Chart review to determine appropriateness for continuum of care services.
  • Create a positive patient care experience by greeting, preparing, exiting and following up with patients for providers according to established competency guidelines, policies and procedures.
  • Create an atmosphere of teamwork by contributing to opportunities to improve employee and customer satisfaction, including self‑development, continuing education and achieving departmental objectives and goals.
  • Provide support and collaborate with physicians and multidisciplinary care teams for best care/results.
  • Ensure accuracy of data collection and reporting for quality initiatives.
  • Maintain accountability for maximizing revenue capture and growth while optimizing cost savings.
  • Communicate opportunities for organizational growth and improvement.
  • Apply evidence‑based practice and scientific knowledge in health care to nursing practice.
  • Demonstrate professional nursing standards defined in the professional practice model and use the ANA Code of Ethics for Nurses to guide responses.
  • Work in a constant state of alertness and safe manner.
  • Perform other duties as assigned.
  • Must live in the Southern to Central Wisconsin Region and be within 1 hour of branch offices. Some onsite coverage is required.
  • Flexible schedule: 8‑4:30, 3 shifts/month minimum, ideally 2‑3 shifts per week, with schedules typically available 2 months in advance.
Qualifications
  • Graduate of an accredited school of nursing or education equivalency for licensing.
  • No experience required.
  • Frequent lifting/carrying and pushing/pulling objects weighing 0‑25 lbs.
  • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent use of hearing and speech to share information through oral communication, including hearing alarms and malfunctioning machinery.
  • Frequent keyboard use/data entry.
  • Occasional bending, stooping, kneeling, squatting, twisting and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25‑50 lbs.
  • Rare climbing.
Required Licenses and Certifications
  • Basic Life Support Health Care Provider (BLS HCP) – American Heart Association (AHA).
  • Registered Professional Nurse (RN) – Illinois Department of Financial and Professional Regulation (IDFPR).
  • Registered Nurse (RN) – Missouri Division of Professional Registration, or RN issued by Compact State and RN IDFPR.
  • Registered Nurse (RN) – Oklahoma Board of Nursing (OBN), or RN issued by Compact State and RN IDFPR.
  • Registered Nurse (RN) – Wisconsin Department of Safety and Professional Services, or RN issued by Compact State.
