Registered Nurse - Nurse Care Manager

As the Registered Nurse - Nurse Care Manager for the Preventive Services Program will assist in meeting the health needs of children and families enrolled in the Preventive Services Program in accordance with state, ACS/OCFS, agency mandates, and medical best practices. The Registered Nurse - Nurse Care Manager will provide health consultation services through family-based home visits, group educational activities, in collaboration with the agency's social work team and community health providers.

Furthermore, this position will provide coordination of family health services and will act as the medical liaison between hospitals, clinics, schools, and other community providers. Other important duties of the Registered Nurse - Nurse Care Manager include health counseling and instruction in preventive, curative, and rehabilitative health matters; coordinating patient care with medical and ancillary care resources; developing or participating in the development of comprehensive community health nursing plans related to such services as: maternal and child health, chronic, and communicable disease control, health care teaching and supervision, and referral and follow-up for infants, children, adolescents, and adults.

Requirements:

· Registered Nurse with current New York State Licensure

· Previous experience providing health care management / community-based nursing

· Previous experience working with a medically fragile population preferred

· Must be willing to travel and make home visits across the entire NYC region (all 5 boroughs)

$110,000 annual

Monday through Friday schedule but visits will likely occur in the early evening.

1st 90 days will be in-person.
