Position SummaryNurse Case Manager is responsible for assessing, planning, implementing and coordinating all case management activities with members to evaluate the medical needs of the member and facilitate the member’s overall wellness. The role develops a proactive course of action to address issues presented to enhance short‑ and long‑term outcomes, and supports integration of services, policies and programs, including network management and clinical coverage policies.
Required Qualifications- RN with current unrestricted Virginia state licensure required.
- 3 years clinical experience (med‑surg, behavioral health)
- Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy.
- Candidate will work from home (WAH) out of their residence in VA.
- Have reliable phone and hard‑wired internet service.
- Work schedules are determined by the Leader based on business needs and are not affected by the work model or location. Mon‑Fri 8‑5 PM EST.
- Colleagues must be reachable via phone, video conference, and email during work hours (8 hours per day) as directed by their Leader.
- Personal responsibilities, including dependent care, must not interfere with work duties. Dependent care during work hours is generally not allowed.
- Be secure and private to maintain confidentiality (HIPAA compliant).
- Meet any additional reasonable requirements set by the Leader or Company.
- A field‑based colleague regularly works in multiple company locations, including client locations, facilities (nursing homes, rehabilitation centers, etc.), member homes, and/or community locations.
- Case management and discharge planning experience.
- Managed care experience.
- Associate’s Degree in Nursing required.
- Prefer a BSN.
- Proficient in multisystem navigation: comfortable using multiple monitors and systems simultaneously to streamline tasks and improve efficiency.
- Effective multitasker: skilled at talking and typing concurrently while managing various responsibilities with focus and accuracy.
- Basic technical troubleshooting: able to set up and connect essential hardware (monitors, keyboards, etc.) and resolve common technical issues independently.
- Digital collaboration & documentation: experienced in electronic documentation and virtual collaboration tools to support remote teamwork and communication.
- Highly organized & results‑oriented: maintains a structured approach to work, ensuring tasks are completed on time and in alignment with performance metrics.
- Metric‑driven performance: demonstrates accountability by consistently meeting or exceeding established goals and expectations.
- Clean & efficient workspace management: keeps work spaces tidy and organized to support productivity and comply with professional standards.
