Registered Nurse (RN) – Home Care Admissions & 90-Day Assessments

Position Type: Part-Time / PRN / Full-Time (Flexible)
Location: Travel to clients’ homes within the service area (Asheboro, High Point, Pleasant Garden. Thomasville, Randleman, Trinity, Troy, Biscoe, Star, Seagrove, Sophia, Archdale, and Ramseur)

We are seeking a compassionate and detail-oriented Registered Nurse (RN) to travel to clients’ homes to complete start-of-care admissions and required 90-day reassessments
. This role is ideal for an RN who enjoys autonomy, flexible scheduling, and working directly with clients in a home care setting.

• Conduct in-home client admissions/start-of-care assessments
• Perform 90-day reassessments and additional assessments as required
• Complete accurate and timely clinical documentation
• Assess client health status, functional abilities, and care needs
• Collaborate with office staff, caregivers, and families to support care plans
• Identify changes in condition and communicate concerns appropriately
• Ensure compliance with company policies, state regulations, and best practices

• Active Registered Nurse (RN) license in good standing
• Minimum of 1 year nursing experience (home care or community-based experience preferred)
• Strong assessment, documentation, and communication skills
• Ability to work independently and manage time effectively
• Reliable transportation and willingness to travel to client homes
• Comfortable using electronic documentation systems
