Facility: Hinds Hospice
Discipline: Nurse Practitioner (NP)

Shift: 8-Hour Days (Flexible)

• Assist the Medical Director with Medicare-required face-to-face evaluations for hospice recertifications
• Perform home visits (homes, SNFs, and care facilities) as needed
• Communicate with Case Managers (CMs) for patients requiring additional follow-up
• Round in the inpatient unit as needed
• Provide clinical education to hospice care team members
• Support both adult and pediatric hospice patients

• Active Nurse Practitioner license
• Hospice experience required
• Palliative care experience required
• Pediatric experience required
• Comfortable working in home-based hospice settings

• Part-time travel assignment
• 1–2 days per week
• Facility is very flexible and open to customizing the schedule
