** Overview:
** The RN ICM works collaboratively with members, families, providers, and the care team to optimize health outcomes, coordinate services, and support members across the care continuum.

The RN will perform telephonic and in-person assessments, identify high-risk conditions, coordinate care plans, and assist with authorizations, referrals, and resource connections while ensuring compliance with regulatory and organizational standards.###

* ** Experience in
** case management, care coordination, or utilization/care review

** clinical assessments** (telephonic and/or face-to-face)
* Experience supporting members with
** chronic conditions, co-morbidities, disabilities, or complex care needs
*** Strong
** communication, clinical judgment, and problem-solving skills
*** Ability to engage collaboratively with
** providers, interdisciplinary teams, and members
*** Experience with health plan or managed care settings preferred
