Position Summary
Harmony Hospice is seeking a compassionate and highly accountable RN Case Manager to join our dedicated interdisciplinary team. The ideal candidate will be deeply familiar with hospice care, demonstrate excellence in clinical documentation, and be proficient in EMR systems such as Well Sky/Kinnser. This position plays a critical role in ensuring patient comfort, care continuity, and compliance through meticulous planning, compassionate interaction, and clear communication.

- Conduct initial and ongoing comprehensive assessments of the patient's physical, functional, psychosocial, and environmental needs.
- Develop, implement, and revise individualized care plans in coordination with the IDG, focusing on end-of-life care goals.
- Ensure all documentation is completed timely, accurately, and in accordance with regulatory and agency standards—especially as it relates to billing and patient care continuity.
- Communicate significant changes in condition or care needs to the physician and other IDG members promptly.
- Provide patient and family education on disease progression, symptom management, medications, and hospice philosophy.
- Deliver skilled nursing care based on best practices in palliation and terminal care.
- Guide and supervise Hospice Aides and LPNs/LVNs with clarity and accountability.
- Provide on‑call coverage and participate in emergency PRN visits as scheduled.
- Support QAPI efforts and contribute to agency goals for quality, compliance, and compassionate care.
- Model professional ethics and promote the agency’s mission and core values at all times.
- Graduate from an accredited nursing program (Diploma, ADN, or BSN).
- Current RN license in Texas or a compact state license per the Board of Nursing.
- 1+ year of hospice experience required; strong preference for candidates with home hospice caseload experience.
- Demonstrated proficiency in Well Sky/Kinnser or similar EMR platforms.
- Strong interpersonal and critical thinking skills; ability to function independently and meet documentation deadlines.
- Reliable transportation, current driver’s license, and valid auto liability insurance required.
- Ability to lift up to 50 lbs and provide care in varying home environments.
