The Registered Nurse (RN) serves as a vital physician extender within a remote, overnight healthcare team
, providing clinical support to providers and patients across multiple states. This part-time overnight position (12:00 AM–8:00 AM) focuses on telephone triage, patient assessment, and clinical coordination during after-hours coverage.
This role follows an alternating overnight weekend schedule
, working Sunday, Monday, Tuesday one week and Thursday, Friday, Saturday the following week
. The RN manages Electronic Health Records (EHR), responds to patient inquiries, delivers evidence-based clinical guidance, and facilitates communication between patients and healthcare professionals to ensure continuity of care and optimal patient outcomes.

Duties and ResponsibilitiesThe duties and responsibilities listed below reflect the general expectations of this role and are not intended to be all-inclusive.
Manage overnight inbound and outbound patient calls for a multi-state nurse triage telephone service.
Utilize Schmitt-Thompson telephone triage protocols to assess patient needs and document interactions accurately within client EHR systems and/or the Call 4 Health platform.
Perform clinical assessments and triage patients via telephone or patient portal in accordance with established protocols.
Provide after-hours clinical and administrative support, including medication refill requests per protocol, escalation of urgent concerns, and notification of on-call providers regarding critical findings.
Coordinate follow-up care for patients post-discharge or post-operative during overnight hours.
Facilitate referrals and assist with prior authorization requests requiring clinical consultation.
Respond to and coordinate requests from external agencies such as hospitals, nursing homes, funeral homes, and Departments of Labor or Motor Vehicles.
Participate in Remote Patient Monitoring (RPM) initiatives by reviewing overnight alerts, tracking vital signs, and providing patient education for chronic conditions including diabetes, hypertension, and COPD.
Screen and qualify patients for clinical trials and conduct overnight follow-ups and patient education as needed.
Perform additional duties as assigned to support overnight operations and patient care delivery.
Education, Licensure, and ExperienceAssociate’s or Bachelor’s Degree in Nursing from an accredited program.
Active Registered Nurse (RN) license in good standing.
Required Licensure:
Active Nurse Licensure Compact (NLC) license,
AND
Active RN licensure in NY, MA, IL, OR, MI, NV, and CA
Minimum of 2–4 years of full-time clinical nursing experience
, preferably in ER, Urgent Care, Adult/Pediatric Care, OB/GYN, Orthopedics, Ambulatory Care, Home Health, or ICU.
Prior telephone triage experience using electronic triage systems strongly preferred.
Qualifications & SkillsAbility to work independently and exercise sound clinical judgment during overnight hours.
Strong collaboration and communication skills in a remote team environment.
Exceptional empathy, professionalism, and patient-centered customer service.
High attention to detail in documentation, including grammar, spelling, and medical terminology.
Strong organizational and time-management skills, including the ability to handle and document a minimum of four calls per hour during peak overnight periods.
Physical RequirementsAbility to sit for extended periods and engage in continuous telephone communication during overnight shifts.
Ability to hear within normal range and wear a headset or earpiece.
Adequate visual acuity to read computer screens and clinical documentation.
Ability to work remotely from a private, HIPAA-compliant home workspace.
Ability to house and utilize company-provided equipment.
Reliable broadband internet access.
Fast-paced, overnight remote healthcare environment supporting after-hours patient care.
Requires flexibility, adaptability, and strong focus during overnight hours.
Occasional overtime or schedule adjustments may be required based on coverage needs.
