We have an exciting opportunity to join our team as a Telephonic Care Nurse, Per Diem.
This 100% remote position is only open to candidates that permanently reside in the following states:

NYU Langone Department of Medicine, Division of Geriatric Medicine and Palliative Care presents an exciting opportunity for nurses passionate about delivering telephonic palliative care management. In this role, the successful candidate will provide telephonic palliative care to Patients Living with Alzheimer’s Disease and other related dementia after discharge from the Emergency Department or Emergency Department Observation Unit. Nurse-led telephonic case management will be conducted for clinical sites across NYU Langone Health.
Key practices will include transitional care, serious illness communication and establishing goals of care, symptom assessment and management, collaborating with key healthcare providers to coordinate care and patient and family needs, and caregiver support. The successful candidate will receive clinical training on telephonic administered care to ensure appropriate patient-caregiver-clinician communication and effective palliative care delivery. The nurse will be clinically supervised by a palliative nurse leader and administratively supervised by the Assistant Director of the ED-LEAD project.
- Quality/Performance Outcomes:
Participates in center-based quality and performance improvement programs consistent with service and departmental programs. - Maintains awareness of current evidence-based practices and uses research data applicable to patient care.
- Participates in clinical inquiry and activities to foster an evidence-based practice.
- Participates in quality improvement programs and activities that promote patient, family/caregiver, physician, and staff satisfaction.
- Participates in weekly learning network interdisciplinary team meetings.
- Adheres to legal and ethical standards in palliative care, ensuring patient confidentiality, informed consent, and compliance with regulations.
- Clinical:
Conduct thorough initial assessments during telephonic consultations, considering the specific needs and conditions of persons living with dementia and their care partners, following program protocols. - Develops individualized care plans based on assessments, prioritizing issues identified in collaboration with the person living with dementia and care partner/caregiver(s).
- Implements interventions identified in the plan of care.
- Collects patient healthcare data and maintains documentation of telephonic consultations, assessments, care plans, and interventions.
- Performs medication reconciliations, ensuring accurate and up-to-date medication lists and addressing any concerns or discrepancies.
- Evaluates patient progress toward attainment of expected outcomes over 6-month periods.
- Educates the patient and care partner/caregiver(s) to facilitate healthy adjustment to new or changed health patterns.
- Considers needs and behaviors of specific patient age and cultural groups in all patient care activities.
- Schedules and conducts follow-up meetings after interdisciplinary team discussions, reviewing care plans and making necessary adjustments.
- Coordination/Leadership:
Coordinates and self-manages patient deliverables weekly. - Supports the missions, philosophy, standards, goals and objectives of the institution and NYU Langone Division of Geriatric Medicine and Palliative Care.
- Demonstrates knowledge of the NYU Langone Health Service Standards and incorporates into the performance of responsibilities.
- Participates in designated activities related to standards and regulations of relevant outside agencies.
- Functions as a respectful member of the healthcare team.
