Overview
NYU Grossman School of Medicine invites applications for a Telephonic Care Nurse, Per Diem. This 100% remote position is open to candidates who permanently reside in the following states:
New York, Vermont, New Hampshire, Pennsylvania, New Jersey, Connecticut, Massachusetts, Rhode Island, Delaware, Maryland, Washington DC, North Carolina, Minnesota, Georgia, Florida, Colorado, Utah, California, Oregon and Washington.
NYU Langone Department of Medicine, Division of Geriatric Medicine and Palliative Care presents an opportunity for nurses passionate about delivering telephonic palliative care management. The nurse will provide telephonic palliative care to patients living with Alzheimer’s Disease and related dementias after discharge from the Emergency Department or Emergency Department Observation Unit. Nurse-led telephonic case management will be conducted for clinical sites across NYU Langone Health.
Key practices include transitional care, serious illness communication and establishing goals of care, symptom assessment and management, coordination with healthcare providers, patient/family needs, and caregiver support. Training will be provided on telephonic administered care; supervision is by a palliative nurse leader and administrative supervision by the Assistant Director of the ED-LEAD project.
- Quality/Performance Outcomes:
- Participate in center-based quality and performance improvement programs.
- Maintain awareness of current evidence-based practices and use research data in patient care.
- Participate in clinical inquiry and activities to foster evidence-based practice.
- Participate in quality improvement activities promoting satisfaction of patients, families, physicians, and staff.
- Participate in weekly learning network interdisciplinary team meetings.
- Adhere to legal and ethical standards in palliative care, ensuring patient confidentiality, informed consent, and regulatory compliance.
- Clinical:
- Conduct thorough initial assessments during telephonic consultations for Persons Living with Dementia and care partners, following program protocols.
- Develop individualized care plans in collaboration with the person living with dementia and care partner/caregiver(s).
- Implement interventions identified in the care plan.
- Collect and document telephonic consultations, assessments, care plans, and interventions accurately.
- Perform medication reconciliations and address discrepancies.
- Evaluate progress toward outcomes over 6-month periods.
- Educate patients and caregiver(s) to promote healthy adjustment to health changes.
- Consider age and cultural needs in all patient care activities.
- Schedule and conduct follow-up meetings after interdisciplinary team discussions and adjust care plans as needed.
- Coordination/Leadership:
- Coordinate and self-manage patient deliverables weekly.
- Support the missions, philosophy, standards, goals and objectives of the institution and NYU Langone Division of Geriatric Medicine and Palliative Care.
- Demonstrate knowledge of NYU Langone Health Service Standards and incorporate into responsibilities.
- Participate in activities related to standards and regulations of relevant outside agencies.
- Function as a respectful member of the health care team and collaborate with interdisciplinary teams to ensure coordinated care.
- Education/Development:
- Attend trainings on delivering telephonically administered palliative care.
- Maintain current knowledge and competence to fulfill responsibilities.
- Develop communication skills for engagement with persons living with dementia, care partner/caregiver(s), and interdisciplinary teams.
- Focus on cultural humility and ongoing professional development.
- Develop proficiency in telehealth technologies and the electronic health record platform.
- A Baccalaureate degree in Nursing is highly preferred; a Diploma or Associate Degree in Nursing requires a minimum of one year experience with plans to earn a baccalaureate degree.
- Current Registered Nurse licensure in New York required; multiple state licensures preferred.
- Palliative or hospice medicine certification greatly preferred; willingness to obtain certification if not currently possessed.
- Willingness to obtain licensure for multiple states if not currently possessed.
- Fluent written and spoken English required.
- Experience with case management, telephonic care, or telemedicine preferred.
- Approximately 15 hours per week with flexibility to schedule telehealth visits.
- Hospice and palliative nursing certification; experience with telephonic care management; multilingual, particularly Spanish.
Qualified candidates must be able to effectively communicate with all levels of the organization.
