Ontario Health Nurse Practitioner in Palliative Care

We are looking for a skilled Nurse Practitioner to ensure high-quality care delivery for patients in Belleville, Trenton, and Picton. In this role, you will provide timely clinical interventions, develop effective care plans, and facilitate communication among healthcare teams. Your dedication to patient-centered practices will be instrumental in navigating the complexities of palliative care services.

Key Responsibilities:

• Respond to urgent clinical issues for complex patients

• Provide home-based clinical assessments and consultations

• Develop and implement interdisciplinary care plans

• Utilize palliative care assessment tools and protocols

• Lead quality improvement initiatives based on evidence

Requirements:

• Current registration with the College of Nurses of Ontario

• Certification from Canadian Nurses Association in palliative care

• At least 2 years of Palliative Care Nursing experience

• Mandatory CPR-Level C & AED certification

• Valid Ontario driver’s license and vehicle access
