Position: RPN - Adult Visiting (Durham Region)

Pay Range

Rate: $26.60 - $32.54 /visit (placement on salary range as per Collective Agreement)

Estimated annual earning potential: $70,000–$120,000. Actual earnings may vary based on availability and service volumes.

We are currently seeking one Registered Practical Nurse to support our Adult Visiting program in the Durham region. The Adult Visiting Program provides specialized in‑home care for individuals aged 18 and older who require complex nursing services and clinical support. Referrals are received through Ontario Health atHome. Nurses in this program are expected to deliver advanced clinical care, including wound care, medication administration and management, VACC, IV start, management of PICC and central lines, ostomy and drain care, chemotherapy management, and more.

In addition, nurses provide ongoing mental health support and regular check‑ins with clients and their families. A valid Ontario driver’s license (G2 or G) and access to a reliable vehicle are required.

Key Areas of Accountability
  • Assess, monitor, and make care planning and care coordination.
  • Evaluate care plan interventions through ongoing reassessment and evaluation results.
  • Ensure the quality of patient care by identifying and acting on factors which affect practice autonomy and delivery of quality care.
  • Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.
  • Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
  • Inform the nursing supervisor of any client activities, relevant concerns, or changes in the client’s condition or care plan, and submit appropriate documentation to the case coordinator.
  • Maintain documentation standards according to the College of Nurses of Ontario.
  • Participate in a standby/on‑call rotation to be available for answering calls and responding, if needed.
  • Participate in team meetings, as required during the stated availability.
Required Qualifications
  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • Membership of your Professional Association preferred (RNAO/WeRPN).
  • RPN diploma/degree from a recognized university or college.
  • Valid First Aid or CPR Certification.
  • Acute Care and Medical‑Surgical nursing experience is an asset.
  • Knowledge of community nursing is an asset.
  • Demonstrate effective communication (written and verbal) and documentation skills.
  • Demonstrate reliable attendance, punctuality, and availability for the role.
  • Commitment to modelling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
  • A valid Ontario driver’s license (G License) and a reliable vehicle for traveling is an asset.
