Senior Nurse Practitioner

The Specialist Nurse Practitioner – Learning Disability Nurse for Community Learning Disability Services will provide direct clinical care to adults with learning disabilities and highly complex needs, assessing, planning, and implementing care across community teams; maintaining associated healthcare records and evaluating both their own care, and that of junior staff where required.

Clinical Leadership & Development
  • Provide clinical leadership across a defined area of Nursing Care and Practice.
  • Offer support and supervision to colleagues and provide specialist advice relevant to clinical practice and expertise.
  • Be responsible for delivering training to team members as well as to teams outside of LD services and the wider Trust.
  • Provide clinical advice and leadership to CLDT managers and profession‑specific advice to the Community Operations Manager.
  • Undertake research and lead on clinical audits relevant to nursing and specialist practice.
  • Provide professional leadership to Community Services and partner with the Learning Disability Matron to ensure robust nursing leadership.
  • Be a valued member of our highly supportive multi‑disciplinary, community learning disability services.
Main Duties
  • Develop positive working relationships to influence the development of best practice.
  • Facilitate collaborative approaches, active presence, and contribution at forums.
  • Formulate, plan, and deliver care for highly complex, sensitive, and often contentious information.
  • Work with peers and partners to develop and support good practice and maintain professional nursing standards.
  • Act as a clinical expert providing highly complex advice, direction, and leadership.
  • Demonstrate understanding of key nursing practice development needs for people with learning disabilities.
  • Involve in practice development and innovation, including development and review of clinical pathways.
  • Support the implementation of approved outcome measures to advance nursing practice.
  • Ensure practice complies with NICE guidelines and reflects best nursing practice.
  • Lead initiatives to motivate, negotiate, and support service improvements, overcoming barriers and resistance.

Job Responsibilities
  • Contribute to critical incident reviews and investigations, supporting the process and identifying learning outcomes.
  • Support the development and implementation of processes to review and disseminate learning from critical incident reviews.
  • Escalate practice concerns to the Lead Nurse/Matron, including sensitive or difficult issues.
  • Lead dissemination and communication of knowledge gained from research and evidence to inform clinical practice.
  • Engage in clinical audit and research and development, developing audits relevant to learning disability nursing practice.
  • Support people with complex health issues secondary to learning disability, including mental illness, physical health, or challenging behaviour.
  • Hold a caseload of complex service users, assess and proactively manage risk.
  • Assess care needs in response to highly complex presentations and develop specialised programmes/packages of care.
