Royal Surrey NHS Foundation Trust is seeking a Band 5 Community Nurse to conduct assessments and deliver care in patients' homes, residential and nursing homes. You will work within a MDT to prevent hospital admissions and manage complex needs, delivering planned and urgent care in community settings.
You will perform risk assessments, catheter care, wound care, and medication administration, while coordinating with GPs and other professionals to ensure safe, patient-centred care.
