The NHS is seeking a Senior Community Nurse to provide expert, patient-centred nursing care to frail, housebound patients across Huntingdon PCN. You will work autonomously, undertake comprehensive assessments, develop care plans and manage a defined caseload with independent clinical decisions.
You will provide clinical leadership within the multidisciplinary team, support colleagues and contribute to service development, quality improvement and population health management.
