Staff Nurse - Community Nursing Service

An opportunity has arisen for full time Staff Nurse contracts within the NE District Nursing Service. The post holder will provide an accessible, appropriate, high quality, culturally sensitive and effective nursing service to all age groups in the community, by working in partnership with service users, care providers and other agencies.

The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience to support and enable the district nursing team to provide a quality nursing service. The post holder will be a registered nurse who will support the district nurse in delivering care to patients within the caseload.

The post holder will be responsible for providing skilled nursing interventions for patients within the caseload, managing delegated activity and providing support to other members of the district nursing team under the supervision of the District Nurse.

All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHSGGC can only issue a Certificate of Sponsorship (CoS) where these are met.

• 2x full‑time positions available (36 hours per week)
• The post holder will work Mon‑Sun various hours between 8am and 8pm and participate in the weekend and public holiday rota.

The post holder is responsible and accountable for assessing, planning and implementation of programmes of care as directed by district nurse. These will be developed in partnership with patients, family, carers and significant others, and will be evidence based taking into consideration the lifestyle, race, gender and cultural background, and factors which impact on physical, mental or emotional health.

The post holder may be required to assume the responsibility of the caseload holder during short periods of sickness, absence and annual leave, seeking support as required from other senior members of the district nursing team.

• Utilise various clinical and environmental risk assessment tools to identify risks and ensure appropriate interventions.
• Report incidents and participate in critical appraisal to reduce recurrences and engage in further investigation as required.
• Support patients, families and carers to achieve optimum health and independence.
• Plan and prioritize workload, delegate work using available resources and make decisions to ensure patients’ needs are met.
• Work within systems to ensure continuation of a quality 24‑hour nursing service.
• May provide care of patients within a Treatment Room or Health Centre setting.
• Use a systematic, flexible and innovative approach to problem‑solving where there is a barrier to understanding or financial constraints.
• Participate in clinical audit and research to ensure continuing improvements in practice and enhance team performance.
• Ensure implementation of local, regional and national policies, procedures and guidelines relevant to the area.
• Provide clinical leadership and be a role model for junior members of the team, representing the values and beliefs of the nursing profession and the organisation.
• Manage day‑to‑day workload delegated by the district nursing sister/charge nurse.
