We're recruiting for an Adult Nurse Case Manager to join a leading specialist complex care provider delivering high-quality care to adults with complex healthcare needs in their own homes across North West London.

This is an excellent opportunity to move away from shift work and into a Monday to Friday role offering autonomy, variety and the opportunity to build long-term relationships with service users while delivering outstanding clinical care.

The Role
As an Adult Nurse Case Manager, you will:

Manage a caseload of adults with complex healthcare needs living within the community
Complete comprehensive clinical assessments and regular care reviews
Develop and review person-centred care plans
Undertake risk assessments and clinical governance activities
Complete competency assessments and clinical supervision for Healthcare Assistants
Support the mobilisation of new care packages
Liaise with families, GPs, Consultants, Commissioners and multidisciplinary teams
Respond to clinical concerns and provide expert clinical guidance
Support CQC compliance and maintain high clinical standards across your caseload

What We're Looking For
Registered Adult Nurse (RGN/RN Adult) with a valid NMC PIN
Experience supporting adults with complex healthcare needs
Clinical skills in one or more of the following:
Tracheostomy care
Ventilation
PEG feeding
Spinal injuries
Neurological conditions
Complex respiratory care

Experience with in community nursing, complex care, case management, district nursing, ICU, HDU or acute hospital settings
Excellent communication and clinical decision-making skills
Full UK driving licence and access to a vehicle
