We're recruiting for an Adult Nurse Case Manager to join a leading specialist complex care provider delivering high-quality care to adults with complex healthcare needs in their own homes across North West London.

This is an excellent opportunity to move away from shift work and into a Monday to Friday role offering autonomy, variety and the opportunity to build long-term relationships with service users while delivering outstanding clinical care.

The Role
As an Adult Nurse Case Manager, you will:

• Manage a caseload of adults with complex healthcare needs living within the community
• Complete comprehensive clinical assessments and regular care reviews
• Develop and review person-centred care plans
• Undertake risk assessments and clinical governance activities
• Complete competency assessments and clinical supervision for Healthcare Assistants
• Support the mobilisation of new care packages
• Liaise with families, GPs, Consultants, Commissioners and multidisciplinary teams
• Respond to clinical concerns and provide expert clinical guidance
• Support CQC compliance and maintain high clinical standards across your caseload

What We're Looking For

• Registered Adult Nurse (RGN/RN Adult) with a valid NMC PIN
• Experience supporting adults with complex healthcare needs
• Clinical skills in one or more of the following:
  • Tracheostomy care
  • Ventilation
  • PEG feeding
  • Spinal injuries
  • Neurological conditions
  • Complex respiratory care
• Experience with in community nursing, complex care, case management, district nursing, ICU, HDU or acute hospital settings
• Excellent communication and clinical decision-making skills
• Full UK driving licence and access to a vehicle
