Provide holistic care as a Licensed Practical Nurse with Vancouver Coastal Health on the Groundspring Primary Care team in Vancouver, BC. Collaborate with interdisciplinary teams to support clients facing health challenges.

Vancouver Coastal Health is seeking a Licensed Practical Nurse for its Groundspring Primary Care team at Lily Lee Community Health Centre. In this vital role, you will practice in line with BCCNM standards, delivering client and family-centered care. Support for clients experiencing health challenges will include outreach and home-based visits within an integrated care model.

Key Responsibilities:

• Collaborate in an interdisciplinary team for client care

• Assess and plan care for primary care clients

• Document evaluations and implement care strategies

• Maintain therapeutic relationships with clients and families

• Seek guidance from RNs/NPs/MDs when required

Requirements:

• Graduation from an accredited Practical Nurse program

• Active registration with BCCNM

• One year of nursing experience in holistic care

• Knowledge of Trauma Informed Practice

• Ability to communicate effectively in diverse settings
