Rate: $26.60-$41.10 /visit (placement on salary range as per Collective Agreement)
Estimated annual earning potential: $80,000–$130,000. Actual earnings may vary based on availability and service volumes
Grounded in our commitment to Client and Family Centred Care the key foci on the RN role is to provide and coordinate home health nursing services to diverse group of individuals, families and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, the program where they provide services, and the Standards of Practice in Community Health Nursing.
We are seeking a full-time Registered Practical Nurse (RPN) or Registered Nurse (RN) to support our Child & Family Program in the Toronto Central region. The successful candidate should have experience providing tracheostomy care, ventilator care, and in G-tube feeding. This is a visiting nurse position and the successful candidate must possess a valid Ontario driver's license (G2 or G) and have access to a reliable vehicle.
The Child and Family Program provide individualized, one-on-one care to clients ranging from newborns to young adults up to 25 years of age. As a visiting nurse, you will deliver home-based nursing care to children with complex medical needs, supporting both their health and overall well-being in a family-centered environment. Key responsibilities of a Child and Family nurse include, but are not limited to, seizure management, medication administration and monitoring, palliative care, G-tube feeding, mechanical ventilator care, tracheostomy care, ongoing assessments, and assistance with personal care such as diaper changes and hygiene needs.
The program also supports continuous professional development through educational opportunities, including access to the Sick Kids Connected Care program, enabling nurses to enhance their clinical knowledge and stay current with best practices.
Key Areas of Accountability
VHA's Registered Nurses and Registered Practical Nurses are required to do the following tasks, but not limited to:
Assess, monitor, and make care planning and care coordination.
Evaluate care plan interventions through on-going reassessment and evaluation results.
Ensure the quality of patient care by identifying and acting on factors which affects practice autonomy and delivery of quality care.
Demonstrate professionalism, leadership, judgement, and accountability in interactions with clients and families.
Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
Inform the nursing supervisor of any client activities, relevant concerns, or changes in the client’s condition or care plan, and submit appropriate documentation to the case coordinator.
Maintain documentation standards according to the College of Nurses of Ontario.
Participate in a standby/on-call rotation to be available for answering calls and respond, if needed.
Participate in team meetings, as required during the stated availability.
Required Qualifications:
Registered and in good standing with the College of Nurses of Ontario (CNO).
