Position: Associate Nurse Manager (Home Care)

Job Summary

The Assistant Nurse Manager (HPC & HPC+) provides clinical leadership, care governance, and operational oversight for Home Personal Care (HPC) and Enhanced Home Personal Care (HPC+) services.

Combining direct community nursing practice with team leadership and multi-agency coordination, this role ensures safe, high-quality, and person-centered care for clients with complex functional, medical, and end-of-life needs within the home environment.

Key Responsibilities Clinical Care & Service Excellence

Deliver direct nursing care and clinical oversight for clients enrolled in HPC and HPC+ programs, including complex cases and palliative/end-of-life care.
Lead care planning, clinical decision-making, and risk escalations in strict accordance with professional scopes of practice and institutional protocols.
Monitor clinical outcomes to maintain high standards of client safety, clinical governance, and care continuity in home settings.
Oversee accurate, timely, and compliant clinical documentation and health records.

Team Leadership & Capacity Building

Support the Nurse Manager in supervising nursing and care personnel, guiding workload allocation, and enforcing clinical standards.
Provide on-the-ground coaching, bedside mentoring, and practice support to nurses and care staff.
Facilitate staff onboarding, clinical competency assessments, and skill development programs.
Proactively manage and elevate complex clinical, staffing, or operational risks to leadership.

Care Coordination & Stakeholder Collaboration

Partner closely with multidisciplinary healthcare teams, community partners, and government agencies to deliver integrated home-based care.
Facilitate case conferences and inter-disciplinary discussions to optimize client care plans.
Seamlessly manage care transitions and step-down care pathways to reduce avoidable hospital readmissions.

Quality, Safety & Governance

Champion quality improvement (QI) initiatives, clinical audits, and service evaluation frameworks.
Enforce clinical risk management, incident reporting, and corrective action plans.
Maintain professional, ethical, and regulatory standards aligned with Singapore Nursing Board (SNB) guidelines.

Job Requirements Education & Registrations

Bachelor's Degree in Nursing (or an equivalent recognized qualification).
Valid registration as a Registered Nurse (RN) with the Singapore Nursing Board (SNB).
Post-basic qualifications or specialized training in Community Nursing, Gerontology, Palliative Care, or Healthcare Leadership are highly advantageous.

Essential Work Experience (Must-Have)

Mandatory Skill Set: Minimum 4 to 6 years of core nursing experience, with direct, hands-on experience in Home Care / Home-Based Nursing settings being essential.
Proven track record in a Senior Staff Nurse, Nurse Clinician, or supervisory capacity within community care or home care ecosystems.
Strong interest and proven capability in clinical leadership and community service development.
