Waypoint Centre for Mental Health Care is a 321-bed academic and teaching hospital that provides specialized mental health, addiction and geriatric care. Located on the shores of Georgian Bay, we serve some of the most complex and disadvantaged individuals in Ontario and are the sole provider of high-secure forensic mental health services in the province.

The Manager of Medical Affairs is currently seeking a full-time Nurse Practitioner. The Nurse Practitioner is a valued member of the inter-professional Integrated Care team and is accountable for the ongoing monitoring of their assigned patient's clinical status, coordinating care, and ensuring continuity across the patient's treatment. This patient group will be part of an innovative step down program designed to provide longitudinal mental health care for individuals requiring sustained support following more intensive services.

The focus is on primary mental health care for individuals requiring clozapine and or LAI supports.

The role includes responsibility for the oversight and management of clinical workflows to ensure timely access, efficient care processes, and optimal patient outcomes. The NP will also lead the development, implementation, and evaluation of clinical protocols, care pathways, and practice standards that support this new clinical stream along with the Medical Director, Integrated Ambulatory Child and Youth Services. As the clinical lead, the NP will guide program development, collaborate with interdisciplinary partners, and provide leadership in shaping this emerging model of ambulatory mental health care.

Please Note:

This position offers a hybrid work environment consisting of in-office and remote work.

  • Provide primary mental health care within a step-down model for patients who no longer require intensive psychiatric follow-up but continue to need ongoing clinical monitoring, treatment support, and care coordination.
  • Support patients receiving long-acting injectable medications, clozapine, or other psychiatric treatments requiring ongoing surveillance by monitoring mental status, physical health needs, medication effectiveness, side effects, and required blood work.
  • Involves patient and families in identifying risk factors and health problems, goal setting and interventions for disease, treatment, prevention and health promotion.
  • Work collaboratively with psychiatry, the medical director, and the interprofessional team to provide continuity of care, with access to psychiatric consultation and re-referral when clinical needs escalates or becomes more complex.
  • Determine the need for, order, and interpret laboratory and diagnostic tests; consult with psychiatry or other physicians when required.
  • Maintain timely and accurate documentation of patient assessments, treatment plans, clinical decisions, follow-up needs, and communication with other care providers to support continuity of care.
  • Contribute to shaping the step-down model, providing input into program design, policy development, clinical pathways, and quality improvement in collaboration with ambulatory mental health leadership.
  • Provide leadership, consultation, and direction to clinicians and care partners to promote consistent care, appropriate referrals, and effective information sharing.
  • Participates in the development and evaluation of policies, protocols and procedures to improve patient care.
