Vancouver Coastal Health (VCH) is proud to be part of the provincial implementation of minimum Nurse to Patient Ratios (mNPRs), an initiative aimed at enhancing nursing practice and strengthening quality of care across the health system. mNPRs set the minimum number of nurses providing care to patients on a given unit. In British Columbia, mNPRs are developed in partnership with the Ministry of Health, the BC Nurses’ Union (BCNU) and health organizations.

Salary Details

The salary range for this position is CAD $32.84/Hr.

Job Summary

Practices in accordance with the standards, scope and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM) for a Licensed Practical Nurse (LPN) as well as within a client and family centred care model. The LPN works collaboratively across multiple sites, clinics, and services, or on outreach/home based visits within an integrated health service delivery model and interdisciplinary care team, to provide holistic care with primary care clients.

The LPN cares for clients and families who are experiencing health challenges across the primary care continuum. In situations where client care needs are not stable or predictable, the LPN seeks direction/guidance from a Registered Nurse (RN), Registered Psychiatric Nurse (RPN), Nurse Practitioner (NP), or Physician (MD). The LPN works closely with the RN/RPN/NP/MD in direct care activities using a team-based care approach, such as assessing, planning, implementing, evaluating and documenting client care as part of the team and collaboratively with other Community Care Services.

The LPN establishes, maintains and enhances therapeutic relationships based on respect. The LPN will support clients and their families to achieve and maintain optimal health. About the Intensive Health Outreach Team (IHOT)

IHOT provides low-barrier interim health care, psychosocial support, and help accessing longer-term health services to people in the Vancouver inner city who are unhoused or newly housed. This is a short-term service that supports people aged 17 and up with immediate health concerns, temporary health care, referral to health services and haem reduction education and supplies.

Qualifications Education & Experience
  • Graduation from a recognized program for Practical Nurses.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • One (1) year of recent related experience or completion of a full final clinical placement in a community setting, working as a Licensed Practical Nurse providing care through a holistic mental health, substance use and/or primary care lens, or an equivalent combination of education, training and experience.
  • Valid BC Driver's License preferred.
Knowledge, Skills & Abilities
  • Comprehensive knowledge of other health care disciplines and their role in client care.
  • Demonstrated ability in dealing with a variety of situations and responsibilities requiring initiative, creativity and professional judgment.
