As a Nurse Practitioner with Carease Health, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. • Provide high-quality preventative care to residents in skilled nursing facilities
• Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation
• Build strong relationships with facilities and maintain a patient-centered approach
• Collaborate with the clinical team to ensure seamless care coordination

• Active NP or PA license in the state of practice
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills

• A competitive annual salary range of $20,000 - $22,000
